Help in filling out the application for Disability (DD) Services in Oregon! 

Join us on Sat, June 6 at 12 noon while we work together to complete the application and answer questions. The same application form is used in each county and for all ages (adults and children.) Hosted by Kim Goldman, ASO Board member, former Service Coordinator and currently an Oregon Needs Assessor for Multnomah County DD Services, and Tobi Rates, Executive Director of ASO.
